School Start-Up Forms

Parents/guardians received an email from YRDSB Update with instructions to complete start-up forms. It is important that these forms are completed as soon as possible at the start of the school year as they contain information and permissions.

The forms can be found in the family dashboard at family.yrdsb.ca.  Click on the arrow by your child’s name and select the start-up forms button. You can choose your language. Please follow these steps for each child.  Forms will automatically populate with your child’s information through the Dashboard. Learn how to create a Family Dashboard account if you do not have one.

The forms are also available online at https://startupforms.yrdsb.ca. You will need your child’s student number.  Tips are available on the Board website. Translation is also available on the website. Please note: You must complete a separate form for each child attending school.

Secondary School Student Forms

Secondary school students can complete the secondary school student form at https://startupforms.yrdsb.ca/students. They will also receive the link in their student GAPPS account. Elementary school students do not need to complete these student forms.

Student Activity Fee

Dear Parents and Guardians,

We are excited to offer your child the chance to make the most of their high school experience at Markham District High School by paying the optional $40 Student Activity Fee (SAF), now available via School Cash Online.

This small fee goes a long way in enriching your student’s life at MDHS. It supports a wide range of events and activities designed to make high school fun and memorable, while also providing discounts and special incentives. By paying the SAF, your child will benefit from:

Discounts on key MDHS events such as Semi-Formals, Fall Fest, SummerFest, and more!

Exclusive free incentives are available only to SAF contributors.

Enhanced opportunities for clubs and extracurricular activities.

For parents of students involved in clubs, it’s important to note that funding for clubs is linked to how many of their members have paid the SAF. The more students who contribute, the more resources their clubs will have to run exciting and enriching programs. Your support can make a real difference in sustaining these opportunities for your child and their peers.

We encourage you to take advantage of this chance to support your child’s high school journey. You can easily contribute via School Cash Online by selecting the $40 Student Activity Fee.

Thank you for being a key part of creating an enriching and unforgettable high school experience at Markham District High School.

Sincerely
Markham District High School Student Council

Parking Lot Safety Reminders:

Please use the parking lot at the front of the school (west parking lot) for all drop offs and pickups.

The fire routes/drop off area is a “no standing zone”. Please park in the parking lot if you arrive early. Vehicles should not be waiting in the drop off loop or in front of the school.

To avoid congestion and to increase the flow of traffic, please do not double park and keep all lanes open to allow cars to exit the parking lot. If you arrive early to pick up your student, please park in a spot in the parking lot to keep the lanes clear.

Please do not block the crosswalks as this is for pedestrian traffic and safe access to the parking lot.

Do not park in an accessible parking space, if you or your passenger do not hold a valid permit.

Be vigilant of pedestrians who are using the sidewalks at the school entrances/exits.

Thank you for your understanding, cooperation and keeping our students safe.
