School Start-Up Forms

Parents/guardians received an email from YRDSB Update with instructions to complete start-up forms. It is important that these forms are completed as soon as possible at the start of the school year as they contain information and permissions.

The forms can be found in the family dashboard at family.yrdsb.ca.  Click on the arrow by your child’s name and select the start-up forms button. You can choose your language. Please follow these steps for each child.  Forms will automatically populate with your child’s information through the Dashboard. Learn how to create a Family Dashboard account if you do not have one.

The forms are also available online at https://startupforms.yrdsb.ca. You will need your child’s student number.  Tips are available on the Board website. Translation is also available on the website. Please note: You must complete a separate form for each child attending school.

Secondary School Student FormsSecondary school students can complete the secondary school student form at https://startupforms.yrdsb.ca/students. They will also receive the link in their student GAPPS account. Elementary school students do not need to complete these student forms.

MEASLES IS SPREADING IN YORK REGION

While the provincial measles outbreak was declared over on October 6, 2025, York Region Public
Health continues to respond to a local outbreak in Thornhill (City of Vaughan), with nine cases currently
linked to the outbreak. Since January 2025, there have been 13 cases of measles in York Region.

WHY IS THIS IMPORTANT?

Measles is a highly contagious virus that can lead to serious complications like pneumonia, brain
swelling, and even death. Symptoms include fever, cough, red eyes, and a rash.
The best way to prevent the measles is through vaccination. Two doses of the Measles, Mumps,
Rubella (MMR) vaccine or Measles, Mumps, Rubella, Varicella (MMRV) vaccine provide about 97%
protection against measles infection. The vaccine is safe and effective.

WHY AM I GETTING THIS LETTER?

Due to the ongoing outbreak, there is an increased risk of measles in York Region. All students
need to have two doses of measles-containing vaccine to attend school in Ontario (or a valid
exemption). If a measles case is identified in a child attending your school, anyone not fully vaccinated
may need to stay home for up to 21 days to prevent this highly contagious virus from spreading.

WHAT CAN I DO?

Check immunization records to make sure you and your child(ren) is/are fully vaccinated
against measles. Children in Ontario typically receive their first dose of a measles vaccine after their
first birthday and their second dose between ages four and six.

WHERE CAN MY CHILD GET VACCINATED?
• York Region Public Health offers free vaccination clinics to all students in JK to Grade 12.
Appointments can be booked here
• Your health care provider, some walk-in clinics and travel clinics also provide these vaccines

REPORT YOUR CHILD’S VACCINATIONS
Report your child’s vaccinations to York Region Public Health. Doctors do not do this for you. This
information is especially important in the event of an outbreak
